This is a small PR with a few objectives

• implement rate limiting for http
• organise middleware
• stop port change in dev

I also refactored the serverTryDesiredPort slightly to improve the typing and (hopefully) simplify the logic.
The indentation changes make it look like the change is larger than it is. Let me know if you have questions

Notes:

• return is necessary after resolve / reject since this do not stop code execution. ie: the promise state cannot be changed, but the remaining code would still run
• what do you think of the rate limits? I believe these should be safe for user scenarios but still block (script gone rogue) issues?
