Small optimization run on this plugin.

• Added loading=lazy to all markup images
• Extracted template to be a function to be more DRY closes make the template HTML output a function so it's dry between shortcode and filter #2
• Documents the VS Code Snippet closes Document the VS Code snippet #12
• Adds markdown-inject to README closes Use markdown-inject for readme  #11
• Adds demo files to make markdown-inject easier (npmignored)
